Insurance Cards and Photo ID

Please bring along the most current insurance card and photo identification (Drivers License preferred).  The beginning of each calender year can bring a new card- make sure you have the right one.  Insurance companies sometimes require pre-authorization.  A quick call to the customer service number of the back of the card can save time.

Copays and Payments

Payment is expected at time of service.  We will bill your insurance company in a timely manner.  We accept debit and credit cards as well as Flex Spending Account cards.

Clothing
Wear loose fitting clothing that allows the therapist to easily view the affected area.  We do have facilities to change if needed.
